Patron Library Card Information
Affiliated/Associated
Who is eligible: Individuals affiliated/associated with a University of Arizona department (including visiting scholars) as designated by the Office of the Vice President for Research, Graduate Studies, and Economic Development (VPR) or the Office of the Vice Provost for Instruction (VPI).
Verification process: Applications for "Affiliate" or "Associate" status should be submitted to the Office of the Vice President for Research, Graduate Studies, and Economic Development (VPR). The principal exceptions are, applications for people involved in instruction alone. These should be submitted to the Office of the Vice Provost for Instruction (VPI) in Administration 501. The contact person in that office is Anne Marie Jones (626-0536). The one further exception concerns nominees from the UA College of Medicine, both in the Tucson and Phoenix areas. These will be processed in that College's Office of the Dean for Academic Affairs (AHSC 2205).
For access to the forms which will be needed it's best to pull them from the HR website at http://www.hr.arizona.edu/forms.php#affiliate.
A letter of request, with the two documents on the above website completed, along with a vitae should be submitted to the appropriate office for review. This entire process can take up to 2 weeks. When the file is reviewed and a determination regarding the category is made, they will forward the paperwork to Systems Control. At that time the data will be entered into the PSOS and any associated benefits will be available. UIS pulls information from PSOS on a regular basis. The library pulls information from UIS every day (excluding holidays).
Library application needed: Affiliates/Associates are downloaded from the University Information System, so the library no longer asks these individuals to fill out an "Application for The University of Arizona Library Card" or bring in any letters to the library.

- Affiliated organizations must be in compliance with university policies pertaining to affiliated organizations.
- Per University policy, email is a mechanism for official communication within the University of Arizona. Official University email accounts are available for all enrolled students, faculty and staff. and must be utilized for library communication.
Alumni
Who is eligible: AZ residents who are members of the University of Arizona Alumni Association.
Verification process: Present proof of AZ residency as well as current alumni association membership card.
Library application needed: Must fill out library card application upon initial purchase, as well as at the time of renewals.
Where to obtain privileges: Express Document Center at the Main Library, or the Information Desk at the Science-Engineering Library. Customers may also obtain new cards and renew existing cards at the Fine Arts Library, however, payment at that Library must be made by a phone call to Express Documents using a debit or credit card. Annual renewal fees must also be paid in person.

Who is eligible: Arizona State University faculty (including adjunct faculty) and students (including ASUSW students).
Verification process:
- Present a current ID card (should have a sticker with current semester expiration date) from ASU.
- Or proof of employment along with a picture ID.
- Or log onto ASU website to access student account to show proof of registration, or class listings to show proof of employment.
Library application needed: Must fill out library card application upon initial registration, as well as at the time of renewals.
Where to obtain privileges: At the information desk at the Main Library, Science-Engineering Library, or Fine Arts Library.

Who is eligible: Arizona businesses
Verification process:
- A letter of request MUST be written on company letterhead including the following:
- the name of the person who will be in possession of the library card when it is not in use
- the names of the three employees authorized to use the card.
- the business mailing address, phone number, and contact person
Library application needed: Must fill out Business Library Card application upon initial purchase, as well as at the time of renewals.

The receipt for payment is brought to the Materials Access Team (MAT) Administrative Assistant (circulation desk).
OR
- Bring the letter and pay appropriate fee in-person at the University of Arizona Main Library, Photocopy (first floor), show the receipt and give the letter to the MAT Administrative Assistant.
- Upon receipt of the request and payment, the MAT Administrative Assistant will process the Library card. The Library card WILL BE MAILED to the business along with library pamphlets.

- Authorized users MUST present company ID card, displaying the employee’s name, a photo id and the Library card when checking out items. If there is a change of authorization, please send a letter with the changes to the Materials Access Team Administrative Assistant via fax or mail.
- One (1) University of Arizona Library Card will be issued per business. Three business employees will be authorization to use this One (1) library card.

Verification process: The library automatically downloads faculty/staff information from the University Information System (UIS) which pulls information from PSOS on a regular basis. The library pulls information from UIS every day (excluding holidays).
Library application needed: Application not needed.

Who is eligible: Spouses or domestic partners of University of Arizona faculty and staff.
Verification process:
- Proof of residency documentation (e.g. utility bill, checks, bank statement or lease agreement with both names on it or separate paper work which has same address; or marriage license)
- Evidence of registration with an entity such as the City of Tucson Domestic Partnership Registry

Special Policies:
- Newly admitted freshman or transfer Honors students are required to officially join Honors during the first 5 weeks of classes. This signifies acceptance of the offer of Honors admission and allows students to keep their Honors benefits. These benefits are outlined in the "benefits" section of the Honors College web site. To officially join, students visit the Honors College web site and fill out the new student form. Students who do not officially join Honors will lose their Honors status and will need to apply for Honors admission as a current UA student via the nomination process.
All current Honors students are required to update their Honors status once each semester, during the first five weeks of classes. Failure to do so may result in the loss of Honors privileges. Please visit the Honors College website for more information.
It should be noted that during the first 21 days of class, your honor's status will NOT be in the library database because the library needs to wait for information to be sent to the Student Information System (SIS) from the Honors College. Please note the checkout limits for undergraduates. Information supplied by the Honors College to SIS will be downloaded on a weekly basis. - Unpaid balances of $2 or more will be sent to the Bursar's office once a month.
