Please read the following regulations. By submitting this form you have agreed to the regulations governing the use of Library Study Carrels. If a renewal is not granted, you will be asked to vacate the carrel.

REGULATIONS GOVERNING USE OF STUDY CARRELS

The Library and University assumes no responsibility for materials left in study carrels.

All carrel assignments are based on Library research needs. Students must be currently enrolled in the university. The University Library reserves the right to reassign unused carrels.

Eating, drinking and smoking are not permitted in carrels. Also, covering the door window is not permitted. Library Staff must be able to visually check each carrel upon closing the library building. Failure to abide by these rules will result in carrel privileges being revoked.

Carrels will be checked periodically for Library materials.

Library materials left in carrels must be checked out. If not checked out, the material will be removed from the carrel. Non-circulating materials cannot be kept in study carrels. If unchecked Library material is found in the carrel, carrel privileges will be revoked.

Carrels must be renewed EACH SEMESTER. Notification of renewal will be sent to you via e-mail toward the end of Fall, Spring and Summer Sessions. Notification will not be sent to your campus or home address.

Carrels may be renewed for no more than 6 consecutive semesters (including summer session), i.e., two consecutive years. After an interval of one full semester without a carrel, re-application will be considered.

When the carrel assignment terminates, you will be notified by email and you must remove your belongings by the date specified, otherwise anything left in the carrel will be removed and the door code will be changed.

Adjacent carrels are in use— please be courteous and maintain minimum noise levels.

Carrel assignments are confidential. Library staff cannot assume paging responsibilities and will not direct others to carrels or share keypad codes.

The Library is also not responsible for personal property.

Once assigned, room changes will not be accommodated except for extenuating circumstances.

Sharing carrels is permitted. However, carrel-mates must also fill out an application. Only those legitimately holding carrel access codes will get renewal and expiration notices. Carrel holders are responsible for notifying anyone else sharing a carrel and responsible for any items removed when door codes are changed.

Most carrels are equipped with an Ethernet connection. Please contact the Computer Center Help Line at 621-HELP or the Digital Library Information Support Team's Helpline at 621-8483 for information on determining your machine's hardware needs and instructions.

FAILURE TO OBSERVE THE REGULATIONS LISTED ABOVE MAY RESULT IN FORFEITURE OF THE RIGHT TO USE A CARREL.
